What caused that plug door to fly off the airplane is, and has been for some time, very clear. Mechanics in the Renton factory performed unauthorized work on that plug door, never receiving a "removal" authorization from QA and neglected to reinstall the door correctly, specifically leaving out the four main attach bolts. This is an issue of manufacturing integrity, and (in my personal experience) happens all too frequently in the Boeing manufacturing facilities. This is a culture problem at Boeing, a dangerous one, and Boeing together with the Dept of Transportation, FAA, NTSB need to map out a way forward. I would like to see Congress get back to the equally important issue of how Boeing got away with designing an engineering monstrosity, the 737 MAX which violated every principle of safe aeronautical engineering. MANY questions about that process and how what basically turned out to be a death trap was certified to fly. It will happen again in all likelihood until and unless all questions are answered (for example WHY was a single point of failure allowed to be incorporated into a critical flight control system? Or WHY was the authority of MCAS increased sharply without the review, or knowledge of the FAA? And SO ON)
