The shaft the clutch slides over should be threaded. Get a bolt and big washer and a lock washer and it will never fall off again! Hope you see this so your family can ride with no issues!!

The set screw for the key actual goes on top of the key. The key is soft metal, so when you tighten it in top of the key, it clamps down in the shaft and keeps the key from sliding out and also keeps it from walking off the shaft. The second set screw is basically a safety measure.
