In the databook , it has been said that , even demons are scared of Sanemi because he is too brutal . He has honed his wind style to another level , that it hurts even more when you get sliced by Sanemi and they would prefer to be killed by anyone but Sanemi.

Yeah ,even i believe that Sanemi would prefer a blind genya than a dead genya ,who wont stop going after demons . Also i believe that , he wants genya to hate him , because all hashira believes they will die fighting demons one day , and sanemi doesnt want genya to go after revenge for him, just like shinobu is doing , for her older sister.

So they explained this last season, but it's explained better in the manga. Genya has the ability to temporarily turn into a demon and gain passive regen by consuming demons. His strength and regen get better the stronger the demon he eats. He has special digestive organs and jaw strength that allow for the transformation. In the demon slayer world, this is a rare ability you have to be born with to allow for this. While never outright said, I've always interpreted this ability as a rare genetic mutation. Demons have been around for over 1000 years, so it makes sense that the human body would try to adapt to demons.

It's such a shame that Mitsuri only got one scene in this episode. I was expecting there to be more scenes with her because she's my favorite Hashira and character in Demon Slayer.
