This song (and some others) breaks my heart every time I hear it. If Freddie didn't share his thoughts about his illness with people (and I gather he didn't), he sure let them out in his songs. His high voice sounds a little thinner to me, but you can't fault him for that. It feels like he was pouring out what remained of his life into his songs.
